On August 11, 2003,we advised the Regional <br /> Board(Howard Hold) of LSCE's plan to submit this permit application to the Environmental <br /> Health Department (EHD) and for tentatively, pending pond access and EHD permit approval, <br /> proceeding with the soil boring construction in early September. It is our understanding from <br /> discussions with Mr. Hold that the Board verbally approves of the planned soil boring <br /> construction program as described in the Annual Monitoring Report for 2002. <br /> In addition to the Well Permit Application, also enclosed are the signed Licensed Contractors <br /> Declaration, the signed Master File Record Information, and two maps showing the location of <br /> the premises and the approximate locations of the planned soil borings. Also enclosed, as <br /> instructed in your e-mail of August 12, are two checks. One check in the amount of$149 is for <br /> the construction($89) and destruction($60) permit fees;the second check in the amount of <br /> $279 is for the three-hour staff time pre-payment. Although LSCE is providing the enclosed <br /> checks for application purposes, future EHD billing should be directed to the attention of SMS <br /> Briners. <br /> Although a letter of approval from the Regional Board was indicated in your e-mail of August <br /> 12,the Regional Board has not provided written confirmation of its acceptance of the planned <br /> work. Please advise if written approval of the Board's concurrence is needed, or if a phone call <br /> directly between yourself and Mr. Hold will suffice. <br /> We appreciate your assistance with this permit application.
