The State Water Resource Control Board(SWRCB)will invoice you directly for activities performed
under the auspices of the Local UST Oversight Program. The goal of the site evaluation performed by Public Health
Services Environmental Health Division (FHS/EHD) is to certify closures of underground tank leakage sites.
Closure certification occurs only after mitigation activities are completed.
In addition, the "Regional Board Staff Recommendations For Initial Evaluation and Investigation of Underground
Tanks" and "Appendix A" reporting format and the LUFT Manual shall be utilized as guidance documents in the
preparation of work plans and/or reports.
PHS/EHD has evaluated the data generated from the investigative work completed to date and has determined that
no further action with regard to the former underground storage tanks at this site is waranted. Before a formal
closure letter can be issued, the existing monitoring well at the site must be properly destroyed. Therefore, by
August 28, 1995, please susbmit a brief well destruction workplan, a permit application (enclosed) completed
by a C-57 Iicensed well driller, and the associated $89 application fee.
If you have any questions or there are any corrections in the responsible party information, contact Linda Turkatte
at (209) 468-3441.
Donna Heran, REHS, Director
Environmental Health Division
" Tvlwk 4.
Linda Turkatte, Senior REHS Diane M.
