PHS/EHD has the following comments for your consideration. <br /> Please be informed that PHS/EHD granted a sampling frequency reduction of monitoring wells, MW2, <br /> MW3, MW6, MW11, MW15 from quarterly to semi-annually, in correspondence dated November 29, 1993. <br /> The report included the results of the June 30, 1994 sampling event during which MW2, MW3, MW6, <br /> MW11, MW12, MW13, MW14, MW15, and GWEI were not sampled. These wells, with the exception of <br /> MW2 and GWE1, were last sampled on December 10, 1993 and should have been sampled during the <br /> June 30, 1994 sampling event. Also, you should be aware that PHS/EHD was not notified prior to the <br /> June 30, 1994 sampling event. PHS/EHD must be notified 48 hours prior to any future site activities and <br /> all monitoring wells shall be sampled prior to September 30, 1994. <br /> PHS/EHD has been informed that construction of the groundwater remediation system has been initiated. <br /> Please submit copies of all the appropriate permits to PHS/EHD immediately and continue to keep <br /> PHS/EHD informed of site activities in a timely manner. <br /> If you have any questions, comments, or wish to schedule a site inspection, please contact Mary Meays, <br /> Senior REHS, of my staff at (209) 468-0337. <br /> Donna Heran, REHS, Director <br /> Environmental Health Division <br /> Mary Meays, Senior REHS Diane M.
