PHS/EHD has the following comments <br /> for your consideration. <br /> The work plan described how monitoring wells, MW12, MW13, and MW14 are to be <br /> destroyed. The plan indicated that the wells will be pressure grouted using <br /> cement/bentonite/sand slurry. The top five feet of casing will then be drilled out and <br /> filled with clean imported material. PHS/EHD does not allow a sand slurry to be used <br /> for pressure grouted monitoring well destructions. Furthermore, the grout must extend <br /> to three feet below grade. <br /> Please be informed that should the contaminant plume migrate in the area these wells <br /> are located, PHS/EHD will require the installation of replacement wells. <br /> Field work may commence upon issuance of a well destruction permit.
