To proceed'with closure of this site, please <br /> submit to the EHD a well destruction permit application along with the required $60.00 <br /> permit fee for each parcel of land on which wells to be destroyed are located, and any` <br /> applicable access agreements or encroachment permits. <br /> Deadline for submittal of the permit applications is November 6, 2009; deadline for <br /> destruction of the groundwater monitoring and remediation wells is December 4, 2009. <br /> Please provide to the EHD certification in writing that all record fee title owners have been <br /> notified of your request for closure pursuant to Section 252.97.15(a) of the Health and <br /> Safety Code. If you are the sole;,property owner, please provide a statement of that fact to <br /> the EHD (form enclosed). <br /> Forty eight hours notice to the EHD is required prior to all site fieldwork.
