State of.California Department of Corrections and Rehabilitation <br /> Memorandum <br /> Date: July 17, 2013 <br /> To: All Staff <br /> Subject: GATE CLEARANCE PROCESS <br /> This memorandum delineates the process for requesting gate clearances. Gate <br /> clearance requests will be submitted to the following primary designee (1.) or the <br /> back-up designee (2) for each state agency: California Department of Corrections <br /> and Rehabilitation (CDCR), California Correctional Health Care Services (CCHCS) <br /> and Department of State Hospitals (DSH). <br /> CDCR: 1. Michael Gomez, Administrative Assistant/ Public Information Officer <br /> (AA/PIO) <br /> 2. Kim Petersen, Community Resources Manager <br /> CCHCS: 1. Deborah Nelson, Administrative Assistant (A) . <br /> 2. Shelly Prater, Office Technician <br /> DSH: 1. Michelle Bolin, Associate Governmental Program Analyst <br /> 2. Sharon Zamora, Hospital Administrator <br /> Gate clearance requests must be submitted to the agency designee at least two (2) <br /> weeks prior to the requested date of visit using the California Health Care Facility <br /> (CHCF) Request for Gate Clearance form (Attachment 1). This form must be <br /> completed in its entirety. All gate clearance requests for medical contract personnel <br /> will be given priority and handled immediately upon receipt. California Law <br /> Enforcement Telecommunications (CLETS) information will be delivered to the <br /> AA/PIO no more than 24 hours after being received. The AA/PIO or the Investigative <br /> Services Unit (ISU) Lieutenant will receive and make a recommendation for approval <br /> or denial within 24 hours of receipt of the CLETS information. In the absence of the <br /> AA/PIO or ISU Lieutenant, the Custody Captain or other designee, as determined by <br /> the Warden, will assume the responsibility. <br /> The agency designee will review the request to ensure that all required information is <br /> included. Once reviewed, the designee will deliver the gate clearance to the AA/PIO. <br /> The AA/PIO will review the request and deliver the form to a Case Records <br /> Supervisor. The Case Records Supervisor or designee will run the CLETS clearance. <br /> Once the CLETS clearance is completed, the gate clearance packet, including the <br /> CLETS clearance report, will be returned to the AA/PIO. The AA/PIO will deliver the <br /> packet to the ISU Lieutenant. The ISU Lieutenant will review the CLETS clearance <br /> and provide the results to the AA/PIO. The AA/PIO will develop a list of approved <br /> and a list of disapproved requests. The AA/PIO will forward the lists of approved and <br /> °r,G�c i s i z Faroe} <br />
