<br /> eyewash stations were corrected onsite.
<br /> 2) Hazardous waste containers in the thirty seven satellite locations were either open, or lacked appropriate
<br /> hazardous properties, physical state, or an accumulation start date, as detailed below. These containers were
<br /> properly labeled at the time of inspection: small warehouse (30 gallon container), Clinic (RCRA waste container),
<br /> oncology (RCRA waste container), GP medication (RCRA waste container), and in the following medical rooms
<br /> (RCRA waste container) A2A, A3A, Al B, A3A, B3B, B7A, B6B, B6A, BSB, BSA, B4B, B4A, B3B, B3A, B2B, B2A,
<br /> B1A, B1 B, CSA, CSB, CSA, C4B, C4A, C3B, C3A, C2B, C2A, C1A, D1A, D1 B, D2B, D3A, and D6A. All observed
<br /> violations pertaining to the aforementioned containers were corrected onsite.
<br /> 3) Ms. E. Medina, who was working in the medical room of building D6B, stated that "chemo waste is put into red
<br /> bin" as well as gloves and other articles utilized in administering chemotherapy drugs. No chemotherapy
<br /> container was present in this medical room at the time of inspection, however, in all other inspected medical
<br /> rooms, a designated chemotherapy container was present if a chemotherapy patient was in presence. According
<br /> to Ms Medina, one patient is currently located in this building who is prescribed a chemotherapy drug"Tretinoin".
<br /> Any person who generates a waste shall determine if the waste is a hazardous waste.
<br /> This facility generated an unknown amount of waste in the last calendar year(Jan-Dec).
<br /> Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials information
<br /> online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov. Be sure to include
<br /> your hazardous waste activity in the Businesses Activities section in CERS in addition to any other relevant
<br /> activities and required fields.
<br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement
<br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting
<br /> paperwork, by November 9, 2014.
