the other attachment is a NEW, blank Well Permit Application for you to keep in your
records as Master's for future copying.
as for the 'completed' application..) have concerns with the well's construction and I have
NOT received the spec sheet and diagram I requested from Sherry a while ago. As
such, I CAN NOT issue the permit until I understand and approve of the construction
specs.
San Joaquin County Well Ordinance requires no less than TWO inches of grout seal
around each casing, whether it's the PVC well casing and the dirt borehole or the PVC in
a steel conductor casing, AND the conductor case in a dirt borehole. Either way,
if cement grout/bentonite grout goes in, it's gotta have NLT 2" between it and the next
thing to it. Looking at the application sent to me, it looks like she's planning on putting a
10" steel conductor casing inside a 8" borehole????? not in this County she isn't. No
consideration for the grout seal, to say nothing about 10 inches going inside 8 inches.
just so you know what I'm looking for, typical hollow stem 2" PVC well casings installed
inside a conductor casing start off with step drilling to total depth of cc placement,
eventually ending up near 15" dirt borehole diameter. Standard steel, treaded, flush-
welded conductor casing at 8.625" OD goes inside this dirt borehole, centralized
somehow. A bentonite pellet seal, about 2-3 feet is installed INSIDE the cc and the
annulus OUTSIDE of the cc is grouted up. This sets for at least the time required per
ASTM cement type times. later, after the grout has set, an 8" hollow stem auger drills
thru the bentonite seal INSIDE the cc to total depth and sets the PVC casing.
Looking down at this from above, outside to center of PVC shows a borehole diameter,
then at least two inches of grout, a steel conductor casing rim, two more inches of grout
and then the PVC casing in the middle. see?
additionally, note who actually SIGNED on page two (Licensed Contractor Declaration-
LCD).. Scott Lewis of CRA signed for WDC's Tom Moreland?? no. consultant's don't
sign FOR drillers on Workers Comp or Drillers license info-complete a new LCD and
send it to me please. Have Bryan Cook send me a list of folks he'll allow to sign these
kinda docs on his behalf. AS of right now, ONLY he can sign for this license..
so, let me know how you plan on drilling/installing these 4 wells. if all are NOT the same
depths, I'll need each depth spec for each material for each well.
Are you truly going to SONIC install the cc? I'll need ODs for your sonic casings before I
approve this. also, she listed two depths.....150' and 175.....that's TD, right? not all 4
wells are the same? are all the cc's 50 deep?
get back to me soon.
I'll need time to process the application if Monday is still going to work for you.
Mike Infurna
San Joaquin County Environmental Health Dept
600 E.
