204.4.4 Inspection of Excavation <br /> V <br /> Excavations must be observed daily by the "competent person". If evidence for potential cave- <br /> ins or slides is apparent, all work in the excavation must be suspended until necessary steps <br /> V <br /> have been taken to safeguard employees. <br /> 204.4.5 Operations of Vehicles Near Excavations <br /> When vehicles or heavy equipment must operate near an excavation, the sides of the excavation <br /> must be shored or braced as necessary to withstand forces exerted by the superimposed load <br /> and the earth pressure. Stop logs or other types of secure barriers must be installed at the <br /> edges of the excavations. <br /> 204.4.6 Bell-Bottom Pier Holes <br /> Employees entering drilled pier holes must be protected by a casing proportioned to sustain the <br /> maximum stresses imposed by earth and water or slurry that extends the full depth of the shaft <br /> and to the bottom of the bell. A safety cage or a shoulder harness secured to a full-time tended <br /> lifeline shall be required for entry and exit. Air monitoring and related requirements of HS- <br /> 205, Confined Space entry, shall be followed. <br /> 204.4.7 Bridges and Walkways <br /> Walkways or bridges with standard guardrails must be provided where employees or equipment <br /> are required or permitted to cross over excavations. Pedestrian walkways shall be of sufficient <br /> strength to permit a vertical deflection of no more than 0.5 inch when a 250-pound weight is <br /> applied to the center of the walkway. All bridges intended for vehicular traffic must be <br /> constructed to withstand twice the load of the heaviest vehicle expected. <br /> 204.4.8 Barricades and Fences- <br /> Excavated areas must be 'completely guarded on all sides with barricades or fences, as <br /> appropriate. If barricades are used, they must be spaced no more than 20 feet apart and shall <br /> not be less than 35 inches high when erected.
