t , <br /> 7.0 <br /> SAMPLE DOCUMENTATION AND CUSTODY- <br /> All pertinent field survey and sampling information will be recorded in a logbook during each <br /> day of the field effort. A logbook will be assigned to each field team and will have a unique <br /> document control number. The logbook will be bound and have consecutively numbered pages. <br /> The Field Team Leader will be responsible for ensuring that sufficient detail is recorded, and <br /> will review the site logbooks daily and initial each page reviewed. <br /> i <br /> No general rules can specify the extent of information that must be entered in a logbook, <br /> however logbooks will contain sufficient information so that one could reconstruct the field <br /> activity without relying on the memory of the field crew. All entries must be made in indelible <br /> ink. Each day's entries will be initialed and dated at the end by the author, and a line will be <br /> drawn through the remainder of the page. All corrections will consist of single line-out deletions <br /> that are initialed. <br /> Strict custody procedures will be'maintained with the field logbooks. While being used in the <br /> field, logbooks will remain with the field team at all times. Upon completion of the field effort, <br /> the logbook will be adequately secured in the office. Photocopies of the logbooks will be used <br /> as working documents. <br /> 9 <br /> Attention to the number of details is necessary to ensure proper handling of the samples once <br /> they are collected in the field. Proper labelling of the samples, maintaining and documenting <br /> sample custody, documenting sample transfer, properly packaging and shipping the samples, and <br /> notifying the proper agents that Ahe samples have been shipped are essential elements of a <br /> successful project. <br /> Sample chain of custody (COC) is a vital aspect of any analytical sampling activity. Samples <br /> and analytical results must be traceable from the time of sample collection until the time the data <br /> are utilized for any major decision. Evidence of collection, shipment, and laboratory receipt <br /> must be documented to accomplish this. The required forms include a chain-of-custody record. <br /> The COC will be filled out in the field during sample collection, will accompany the samples <br /> rr at all times, and distributed as discussed in the Sampling Plan. <br /> L .. <br /> In order to verify sample integrity; the COC serves as written conclusive proof that samples are <br /> ! taken, transferred, prepared and analyzed in an unbroken chain. By definition, samples are in <br /> custody if they are: in the possession of an authorized individual; in the field of vision of an <br /> authorized individual; or in.a secure area or a locked container. <br /> rThe field and laboratory sample custodians are responsible for sample custody during all sample <br /> handling activities.
