Hammer Lane, Stockton, CA 95210 page 2 <br /> • Please submit a copy of an access agreement signed by each property owner <br /> authorizing your entry to each of the other private properties along with your EHD Well <br /> and Boring Permit Applications. <br /> • The EHD has reviewed the data for the wells at this site and has determined that wells, <br /> MW-1 & VW-1 & 2, VW-3 & 4, VW-5 & 6, VW-7 & 8, SW-1, MW-2, MW-3, MW-4, <br /> MW-5, and MW-10 were either contaminated at the last reported sampling event or are <br /> in an area of residual contamination, as depicted by recent soil and groundwater iso- <br /> concentration figures provided to the EHD for closure review. These wells do not <br /> qualify for pressure grouting and must have the contents removed. <br /> • Currently there are four wells with multiple casings installed in one borehole. Each of <br /> these multi-cased (nested) wells are considered as one well for permit fee calculations. <br /> Double and triple nested wells proposed for over-bore must use an auger of an <br /> adequate size that when centered on the deepest casing, is capable of reaching the <br /> outside most radius of the original borehole. For single cased wells that are to have <br /> the contents removed by over-boring the wells, please plan on using the same auger <br /> diameter as used for installation. <br /> The permit fee for the destruction of each well is $125, along with a $375 Inspection Activity <br /> fee per application. Only MW-2 is located on property not owned by Shadrall Hammer Lane <br /> LP and this permit application requires a $500 fee. A review of the site indicates that 21 other <br /> casings are present in 16 boreholes. These 16 boreholes (wells), whether located on or off of <br /> the gas station property, can be included on one permit application since the properties are all <br /> owned by a single landowner. The permit fee for 16 wells is $2375. Please submit two <br /> separate and signed EHD Well and Boring Permit applications with the fee for each. A <br /> completed and signed Master File Record form is also to be submitted with the permit <br /> applications. <br /> Additionally, the vapor or air sparge transmission lines/piping are to be either removed before <br /> well destruction activities or grouted in-place with neat Portland cement at the time of the well <br /> destructions. The EHD approves the dismantling and removal of the remediation equipment <br /> still onsite.
