• Violation#5, Inventory is not complete/accurate. <br /> 2. Carbon Dioxide—the size (volume) of the largest container is greater than the 50 pounds reported; <br /> the maximum daily amount cannot be less than the size of the largest container,the average daily <br /> amount is wrongly reported as .13 pounds, and the days on site (365) has not been completed. <br /> • Violation#6 has been closed however,the multiple maps must be numbered, i.e. Map#1, Map#2. Please, number <br /> and upload the maps. <br /> Violation#7, Emergency Response/Contingency Plan is not complete. <br /> - The plans were lacking procedures for the immediate notification the Environmental Health <br /> Department (EHD) at 209-468-3420 and the California State Warning Center/Cal OES at 1-800-852- <br /> 7550. <br /> - Note,the Consolidated Emergency Response/Contingency Plan Template may be used to satisfy <br /> requirements that Hazardous Materials Business Plans (HMBP) contain emergency response plans, <br /> procedures, and employee training in the event of a reportable/threatened hazardous material release <br /> (see attached). <br /> • Violation#7, Emergency Response/Contingency Plan is not complete (see attached Emergency <br /> Response/Contingency Plan Template). <br /> 3.
