Category 10 <br /> lists your facility(Standard Industrial Classification Code 4221) as one of those facilities required to <br /> obtain coverage under the General Permit. Attachment 2 is a Notice of Intent (NOI)which must be filed <br /> to obtain coverage under the General Permit. Therefore, we request that you submit by 26 October 2001 <br /> to the State Board the following: (1) an NOI with all applicable sections filled out and signed by the <br /> owner/operator; (2) a check made out to the "State Water Resources Control Board"; and (3) a site map <br /> displaying the layout'of premises.
