PHS/EHD has the following comments for your consideration. <br /> PHS/EHD requested additional soil investigation, in correspondence dated October 16, 1992, in the areas <br /> t of soil borings, B-12 (waste oil tank) B-13 (dispenser island), and beneath the oil-water separator. <br /> I PHS/EHD also requested a downgradient monitoring well to establish the boundaries of soil and/or <br /> groundwater contamination. <br /> The work plan proposed four soil borings drilled to approximately 48 feet and one soil boring drilled to 60 <br /> feet, to be converted into monitoring well, MW5. There were two soil borings proposed to investigate the <br /> dispenser island. Consideration should be given to possibly converting one of these soil borings into a <br /> monitoring well, providing a downgradient well to monitor contamination evidenced by soil boring B-13. <br /> The work plan did not include investigation of the oil-water separator. Additional investigation of this <br /> area will be necessary. <br /> Enclosed is the accepted permit #93-0202. Please contact PHS/EHD 48 hours in advance of site work to <br /> schedule an inspection. Please note PHS/EHD inspections are an important aspect of local regulatory <br /> oversight.
