76 Service Station No. 11193 Page 2 <br /> 3202 West Hammer Lane <br /> Stockton, California <br /> limits, may be destroyed by pressure grouting from the bottom upward, while leaving <br /> the well casing in place. The top three feet or more of the well shall be drilled out with <br /> an auger that is at least one foot larger than the auger originally utilized to install the <br /> well, and the soil around the cutoff casing removed to a depth of an additional six <br /> inches below the new `top' of the cutoff casing. The grout sealant shall spill out of the <br /> top of the well casing to form a mushroom cap seal, with at least 12 inches of neat <br /> cement grout poured 6 inches above and 6 inches below the top of the cutoff casing. <br /> • All groundwater displaced to the surface during grout installation shall be pumped out <br /> of the excavation to allow for the EHD inspection and verification that the well was <br /> properly destroyed. Contaminated groundwater pumped out of the excavation shall be <br /> containerized for proper disposal. <br /> • This site has 3 wells located off-site in the City of Stockton right-of-way. Please submit <br /> a copy of the valid encroachment permit and an access agreement for the one well <br /> located on parcel number 08240001 when submitting the EHD Well and Boring Permit <br /> Application forms for these wells. <br /> After the EHD has approved the well destruction scope of work proposal, you may submit an <br /> EHD Well and Boring Permit Application form and the appropriate fees for each parcel. The <br /> permit fee for the destruction of each well is $125.00, along with a $375.00 Inspection Activity <br /> fee. A review of the site records indicate that 23 wells currently were installed. A fee of <br /> $4,000.00 (23 x$125.00 + $375.00 x 3) will be required for the well destruction permits. <br /> Please provide the EHD certification in writing of a complete list of current record fee title <br /> owners and their mailing addresses pursuant to section 25297.15 (a) of Chapter 6.7 of the <br /> Health and Safety Code.
