Parkinson's 1-5 ARCO Service Station Page 2 of 3 <br /> 3250 West Hammer Lane May 30, 2013 <br /> Stockto-i California 95209 <br /> Limits, and are not in areas of known or suspected soil contamination, may be <br /> destroyed by pressure grouting from the bottom upward, while leaving the well casing in <br /> place. The top three feet or more of the well shall be drilled out with an auger that is at <br /> Bast one foot larger in diameter than the auger originally utilized to install the well, and <br /> tie soil around the cut-off casing removed to a depth of an additional six inches below <br /> tie new `top' of the cut-off casing. The grout sealant shall spill out of the top of the well <br /> casing to form a mushroom cap seal, with at least 12 inches of neat cement grout <br /> poured 6 inches above and 6 inches below the top of the cut-off casing. <br /> • END records indicate the presence of eight 'nested' injection wells [IW-1(A/B) through <br /> I`N-4(A/B)] installed using eight-inch augers. The EHD has found destruction results of <br /> nested wells are best obtained when an auger larger than the original auger is used and <br /> centered on the deepest casing (which is off-center in the original borehole). The auger <br /> needs to be sized sufficiently to reach out and remove all well casings, screens, filter <br /> packs, and grout sealant installed during construction. <br /> • All groundwater displaced to the surface during grout installation shall be pumped out of <br /> the excavation to allow for the EHD inspection and verification that the well has been <br /> properly destroyed. Contaminated groundwater pumped out of the excavation shall be <br /> containerized for proper disposal. <br /> • This site has four wells located off-site in a private road. Please submit a copy of the <br /> access agreement when submitting the EHD Well and Boring Permit Application form <br /> for these wells. <br /> After the EHD has approved the well destruction scope of work proposal, you may submit an <br /> EHD Well and Boring Permit Application form, Master File Record form, access agreement for <br /> wells located in the private road, and the appropriate fees for each parcel. The permit fee for <br /> the destruction of each well is $125, and the Inspection Activity fee is $375 per parcel. A review <br /> of the site records indicates that 7 monitoring wells and 4 injection wells (IW-1 through IW-4) <br /> currently are installed. A fee of $2,125.00 (11 wells x $125.00 + $375.00 x 2) will be required <br /> for the well destruction permits. <br /> To determine the fees for nested wells (including IWA, IW-2, IW-3 and IW-4), each nested well <br /> is considered as one well for permitting and fee purposes. <br /> Please provide the EHD certification in writing that all record owners of fee title to the above <br /> referenced site have been notified of your request for closure. This is in accordance with <br /> Section 25297.15 (a) of the California Health and Safety Code. Submit a complete list of record <br /> owners of fee title and their mailing addresses to the EHD.
