The top three feet or more of the well shall be drilled out with an auger that is at Bast one foot larger in diameter than the auger originally utilized to install the well, and tie soil around the cut-off casing removed to a depth of an additional six inches below tie new `top' of the cut-off casing. The grout sealant shall spill out of the top of the well casing to form a mushroom cap seal, with at least 12 inches of neat cement grout poured 6 inches above and 6 inches below the top of the cut-off casing. • END records indicate the presence of eight 'nested' injection wells [IW-1(A/B) through I`N-4(A/B)] installed using eight-inch augers. The EHD has found destruction results of nested wells are best obtained when an auger larger than the original auger is used and centered on the deepest casing (which is off-center in the original borehole). The auger needs to be sized sufficiently to reach out and remove all well casings, screens, filter packs, and grout sealant installed during construction. • All groundwater displaced to the surface during grout installation shall be pumped out of the excavation to allow for the EHD inspection and verification that the well has been properly destroyed. Contaminated groundwater pumped out of the excavation shall be containerized for proper disposal. • This site has four wells located off-site in a private road. Please submit a copy of the access agreement when submitting the EHD Well and Boring Permit Application form for these wells. After the EHD has approved the well destruction scope of work proposal, you may submit an EHD Well and Boring Permit Application form, Master File Record form, access agreement for wells located in the private road, and the appropriate fees for each parcel. The permit fee for the destruction of each well is $125, and the Inspection Activity fee is $375 per parcel. A review of the site records indicates that 7 monitoring wells and 4 injection wells (IW-1 through IW-4) currently are installed. A fee of $2,125.00 (11 wells x $125.00 + $375.00 x 2) will be required for the well destruction permits. To determine the fees for nested wells (including IWA, IW-2, IW-3 and IW-4), each nested well is considered as one well for permitting and fee purposes. Please provide the EHD certification in writing that all record owners of fee title to the above referenced site have been notified of your request for closure. This is in accordance with Section 25297.15 (a) of the California Health and Safety Code. Submit a complete list of record owners of fee title and their mailing addresses to the EHD.
