Michael Karvelot and Wayne Adachi: The San Joaquin County Environmental Health Department (EHD) has reviewed Well Destruction Work Plan (work plan), dated 17 April 2012, prepared and submitted by your consulting firm, ATC Associates Inc. (ATC) for the above-referenced site. The work plan was submitted in response to the EHD letter dated 01 March 2012 directing a work plan be submitted to destroy the nine on- and off-site monitoring wells. In the work plan, ATC proposes to over-drill monitoring wells MW-1 R, MW-2 and MW-5, and pressure grout MW-3, MW-4, MW-6 through MW-8, and MW-11. The EHD has reviewed historical soil and groundwater analytical results for this site and approves the methods proposed to destroy each monitoring well. Although it is not mentioned in the work plan, the EHD directs that 25 pounds per square inch (psi) of liquid grout pressure be applied for ten minutes to the wells being pressure grouted, followed by over-drilling the well to approximately three feet below surface grade (bsg) using hollow stem augers to create a hole at least a foot larger in diameter than the originally drilled hole. Since the wells were all installed using eight-inch augers, the augers used to over-drill the wells to approximately three feet bsg must be at least twenty inches in diameter. Also use a tremie pipe when grouting the boreholes after over-drilling the three monitoring wells. With these conditions, the EHD approves the work plan. Please submit a copy of the encroachment permit issued by the City of Stockton, access agreement for the two wells located on an adjacent parcel, Master File Record form, well destruction permit applications for each parcel, and permit/inspection fees totaling $2,250.00 to the EHD. Well destructions must be scheduled with the EHD at least a week prior to the fieldwork. Please note that the EHD has moved.
