Michael Karvelot and Wayne Adachi: <br /> The San Joaquin County Environmental Health Department (EHD) has reviewed Well <br /> Destruction Work Plan (work plan), dated 17 April 2012, prepared and submitted by your <br /> consulting firm, ATC Associates Inc. (ATC) for the above-referenced site. The work plan <br /> was submitted in response to the EHD letter dated 01 March 2012 directing a work plan be <br /> submitted to destroy the nine on- and off-site monitoring wells. <br /> In the work plan, ATC proposes to over-drill monitoring wells MW-1 R, MW-2 and MW-5, and <br /> pressure grout MW-3, MW-4, MW-6 through MW-8, and MW-11. The EHD has reviewed <br /> historical soil and groundwater analytical results for this site and approves the methods <br /> proposed to destroy each monitoring well. Although it is not mentioned in the work plan, the <br /> EHD directs that 25 pounds per square inch (psi) of liquid grout pressure be applied for ten <br /> minutes to the wells being pressure grouted, followed by over-drilling the well to <br /> approximately three feet below surface grade (bsg) using hollow stem augers to create a <br /> hole at least a foot larger in diameter than the originally drilled hole. Since the wells were all <br /> installed using eight-inch augers, the augers used to over-drill the wells to approximately <br /> three feet bsg must be at least twenty inches in diameter. Also use a tremie pipe when <br /> grouting the boreholes after over-drilling the three monitoring wells. With these conditions, <br /> the EHD approves the work plan. <br /> Please submit a copy of the encroachment permit issued by the City of Stockton, access <br /> agreement for the two wells located on an adjacent parcel, Master File Record form, well <br /> destruction permit applications for each parcel, and permit/inspection fees totaling $2,250.00 <br /> to the EHD. Well destructions must be scheduled with the EHD at least a week prior to the <br /> fieldwork. <br /> Please note that the EHD has moved. Future correspondence and permits should be <br /> submitted to 1868 East Hazelton Avenue, Stockton. <br />
