After reviewing the documents, we find that <br /> the following additional information isstill needed to determine your eligibility for <br /> placement on the Priority List: <br /> • Page one (1) of your claim application lists Beneto Inc as the UST Owner & UST <br /> Operator. This doesn't correlate with the names of UST Owner(s)/ Operator(s) as <br /> listed on page three (3) and the revised page five (5) of your claim application. <br /> Please clarify and revise these three pages so that the name(s) is (are) consistent <br /> between pages. <br /> • On page six (6) of your claim application you were asked to complete the financial <br /> responsibility mechanism by checking an additional box for the deductible. Because <br /> you have provided a Letter From Chief Financial Officer, I checked the box (number <br /> 9) for Chief Financial Officer Letter for you. <br /> • The Certification of Financial Responsibility (CFR) and Letter From Chief Financial <br /> Officer forms are both being for completion/correction. The Fund requires an <br /> authorized signature for these documents. In addition, the CFR requires two <br /> signatures; one of claimant and one of a witness or notary (this only had one <br /> signature, which appears to be the signature of a witness). If someone other than <br /> claimant signs Fund documents, we require a Power of Attorney form to be on file <br /> with the Fund. <br /> • Claimant previously submitted a Power of Attorney form but it was incomplete.
