CLOSURE CHECKLIST <br /> v <br /> Table 1 of Appendix—Reports, Tri-Regional Board Staff Recommendations for <br /> Preliminary Investigation and Evaluation of Underground Tank Sites provides a checklist <br /> for No Further Action Requests at underground tank sites. The information required is <br /> presented below. <br /> k.. <br /> 1. Distance to production wells for municipal, domestic, agriculture, industry, and other <br /> uses within 2,000 feet of the site. <br /> There are five production wells located within 2,000 feet of the site. Three irrigation <br /> wells are located on the cemetery grounds. With respect to the location of the former <br /> underground storage tanks (USTs), Catholic Cemetery Irrigation Well #1 is located <br /> ' 900 feet to the south southeast, Well #2 is located 50 feet to the south, and Well #3 <br /> is located 600 feet to the north. In addition, a drinking water well serving St. <br /> Joseph's Hospital is located approximately 500 feet to the west. Finally, a domestic <br /> well is located 2,000 feet to the west at 1821 North Hunter Street. The Catholic <br /> Cemetery irrigation well locations are shown on Figure 4 and a complete water well <br /> survey is provided as Appendix E. <br /> 2. Site maps, to scale of area impacted showing locations of any former and existing <br /> tank systems, excavation contours and sample locations, boring and monitoring well <br /> elevation contours, gradients, and nearby surface waters, buildings, streets, and <br /> subsurface utilities. <br /> Elevation contours are included on the site location map (Figure 1). The former <br /> USTs, soil borings and monitoring wells locations are shown on the site plan and <br /> expanded site plan (Figures 2 and 3). Nearby streets and buildings are shown on <br /> t_. the water supply well location map (Figure 4). Groundwater gradient maps for both <br /> shallow and deep constructed wells are presented as Figures 6 and 7. <br /> 3. Figures depicting lithology(cross-section), and treatment system diagrams. <br /> Site lithology for cross section C-C' is shown on Figure 12, and a lateral view of <br /> cross section C-C' is included on Figure 3. The former vapor extraction system <br /> capture zones and system layout are shown on Figure 5. <br />
