Harlan Road 5300 Penny Lane <br /> Lathrop, CA 95330 Pleasanton, CA 94588 <br /> Subject: Joe's Place Site Code: 231585 <br /> 15600 Harlan Road <br /> Lathrop, CA <br /> The San Joaquin County Environmental Health Department (EHD) is requesting concurrence <br /> with a "no further action required" (closure) determination from the Central Valley Regional <br /> Water Quality Control Board. This is to notify you that Section 25297.15 (a) in Chapter 6.7 of <br /> the Health and Safety Code includes a requirement that EHD (local agency) not issue a closure <br /> letter, or make a determination that no 'further action is required with respect to "hazardous <br /> substance" underground storage tank release sites unless all current record owner of fee title to <br /> the site have been notified of the proposed action by the primary or active responsible party. <br /> Please provide EHD certification in writing that all record fee title owners have been notified of <br /> the request for closure. If you are the sole property owner, provide a statement of that fact to <br /> EHD, to fulfill this requirement(form enclosed), <br /> In addition, prior to a closure/no further action required letter being provided to you, the <br /> monitoring wells installed during the site investigation must be destroyed under EHD permit and <br /> inspection. After reviewing the data on these wells, MW 1-MW4, MW7-MW9, MW 11, MW 12C, <br /> MW1 A1B, MW13C, MW15A, MWO-1X, MW02 and MW03 must be drilled out and grouted. <br /> Wells MW5; MW6, MW10, MW14A, MW14B, MW15B and MW01 can be pressure grouted with <br /> the required below surface cut off and mushroom cap as stated in the San Joaquin County Well <br /> Standards. Any deviations from the well standards must be justified.
