This log shall be used to record the following: <br /> • Instrument calibrations <br /> 1 <br /> • Work zones established each day - list location and activity for each. <br /> • Breathing zone and borehole readings. <br /> - type of instrument/reading <br /> - location <br /> - time of measurement <br /> - activity <br /> - actions taken in response to elevated levels <br /> • Any unusual;occurrences (injuries, incidents of non-compliance with ) <br /> HASP, instrument alarms sounding, etc.) or safety concerns. <br /> • Upgrades/downgrades in PPE. The duration of use of Level C equipment <br /> shall be noted. <br /> • Weather conditions each day (temperature, wind and precipitation) are <br /> sufficient. <br /> 10.0 WORKER DECONTAMINATION <br /> Decontamination is required each time persons leave a work zone and there is:reason <br /> to suspect soil contamination. Decontamination for this type of work relies heavily on <br /> the use of disposable PPE. ' <br /> When personnel move between active work zones, minimal decontamination will be <br /> required.
