Project Manager, FSS and Field Staff. <br /> 12.4 Work Site Access <br /> Access within a five-foot radius of any on-site operation is prohibited to all but <br /> Avalon authorized field personnel and subcontractors. <br /> 13.0 HEALTH AND SAFETY REQUIREMENTS <br /> 13.1 Medical Surveillance <br /> All subcontractor personnel and visitors who intend to enter the exclusion <br /> zones or decontamination areas shall participate in a medical surveillance <br /> program that meets the requirements of 29 CFR 1910.120. <br /> All on-site personnel are included in the corporate medical monitoring program <br /> and receive physical examination on an annual basis. <br /> Additional medical monitoring may be required if exposure or potential <br /> exposures are identified by the FSS for which such monitoring is deemed <br /> necessary. The content of this monitoring will be determined by the FSS and <br /> consulting physician. <br /> The petrochemicals typical of these facilities can affect specific organ systems <br /> producing characteristic health effects. The medical evaluation will, therefore, <br /> focus on the liver, kidney, nervous system, blood systems, skin and lung <br /> functions. Laboratory testing will include complete blood count and applicable <br /> kidney and liver-function tests. Other tests include skin examination. <br /> 13.2 Training Requirements <br /> All personnel (including subcontractors) whose work requires entry into <br /> exclusion zones, decontamination areas or otherwise present potential exposure <br /> to health or safety hazards associated with this project, shall meet the training <br /> requirements set forth in 29 CFR 1910.120, the OSHA Hazardous Waste <br /> Operations and Emergency Response Standard. <br /> Personnel shall not be permitted to participate in or supervise activities in the <br /> exclusion or decontamination zones until they have completed the specified <br /> training.
