San Joaquin Motor Pool Facility Health and Safety Plan <br /> IPersonnel Injury Upon notification of an injury, the SHSC will assess the nature of the <br /> injury If the cause of the injury or loss of the injured person does not affect the <br /> performance of site personnel, operations may continue, with the appropriate on-site field <br /> personnel initiating the appropriate first aid and necessary follow-up If the injury <br /> increases the risk to others, all field personnel shall move to a safe location until further <br /> instructions Activities on site will stop until the added risk is removed or minimized <br /> Fire/Explosion: In the event of a fire or explosion on-site, the fire department will be <br /> summoned All site personnel shall assemble at the decontamination line,then move to a <br /> safe distance from the involved area <br /> Personal Protective Equipment Failure: If any site worker experiences a failure or <br /> alteration of protective equipment that affects the protection factor,that person shall <br /> immediately leave the exclusion zone Re-entry shall not be permitted until the <br /> equipment has been repaired or replaced <br /> Other Equipment Failure If any other equipment on site fails to operate properly, the <br /> SHSC shall be notified and determine the effect of this failure prior to continuing <br /> . operations on-site If the failure affects the safety of personnel or prevents completion of <br /> the work plan tasks, all personnel shall leave the exclusion zone until the situation is <br /> evaluated and appropriate actions taken <br /> 6.4 Emergency Response Follow-Up <br /> The SHSC shall submit an incident report to the Project Manager The report shall <br /> include the following <br /> • Chronological history of the incident, <br /> • Facts about the incident and when they became available, <br /> • Titles and names of personnel, <br /> • Actions decisions made and by whom, order given, to whom, by whom and <br /> when Action taken who did what, when, where, and how, <br /> • Possible exposure of site personnel, <br /> • History of all injuries or illnesses during or as a result of the emergency <br /> 11 <br />
