Contractor shall contact USA for location of underground utilities. <br /> 2. The monitoring well proposed on Old Harlan Road shall be located within the sidewalk <br /> area.The sidewalk in the area of the well shall be removed and replaced to existing <br /> expansion joints. The monitoring well shall in a concrete box with a concrete lid <br /> approved by the Public Works Director. <br /> 3. The removal of the existing well (MW-7)shall be done under a permit by County Health <br /> and the following: <br /> A. Drilling operations on Louise Avenue shall be done on a Sunday morning <br /> between the hours of 6 a.m. and 9 a.m.. <br /> B. The bore hole shall filled to three feet from the finish surface to County Health <br /> standards(if filled with concrete,plate and allow 72 hours cure time),then a two- <br /> sack slurry,shall be place in the next two feet. The bore hole shall then be plated <br /> and concrete allowed to cure a minimum of 72 hours before paving. Existing <br /> pavement shall'be saw-cut to neat edge and replaced with 12"of 1/2"Type A <br /> asphalt. <br /> 4: The monitoring well proposed on Louise Avenue,shall be located outside of the traveled <br /> way (location to be approved by the Public Works Director). The well shall be in an <br /> approved traffic box.(Submittal Required). Upon City's Request,this well shall be <br /> relocated at no cost the City. <br /> 5. Payment of a deposit of$2000 against inspection fees. <br />
