Additionally,PHS-EHD directed that the QMR for the First Quarter-1999 be submitted by April 26, <br /> 1999. This QMR and the QMR for the Fourth Quarter-1998 have not been received at PHS-EHD. <br /> You are not in compliance with directives issued by PHS-EHD. UST sites are required to be in compliance <br /> with directives issued by PHS-EHD in order to become or remain eligible to receive reimbursement from the State <br /> Water Resources Control Board(SWRCB)Clean Up Fund(CUF)- Failure to comply with PHS-EHD directives <br /> may also result in a referral to the San Joaquin County District Attorney's Environmental Prosecutions Unit(SJC- <br /> DA-EPU) for formal enforcement action. <br /> The SWRCB-CG'F allows 90 days for responsible;parties to come into compliance with directives issued by PHS- <br /> EHD. This is to notify you that you have until June 10, 1999(ninety days from the original directive)to <br /> submit the Quarterly Monitoring Reports. <br /> PHS-EHD will notify the SWRCB-CUF of the status of this site. A review of the site and compliance issues will <br /> be conducted after June 10, 1999. If QMRs are not received prior to the next quarterly monitoring event,PHS- <br /> EI-ID will recommend that the reimbursement claim for this site be rejected.
