T:. <br /> r. <br /> GROUNDWATER MONITORING WELL INSTALLATION AND <br /> OZONE MICROSPARGE SYSTEM INSTALLATION AND STARTUP REPORT <br /> For <br /> ConocoPhillips (76) Service Station No. 11195 <br /> 16500 South Interstate 5 <br /> Lathrop, California <br /> Report No. 140193.14-2 <br /> 1.0 INTRODUCTION <br /> At the request of ConocoPhillips, Gettler-Ryan Inc. (GR),has prepared this report documenting the <br /> installation of four off-site groundwater monitoring wells and the installation and startup of an <br /> ozone microsparge remedial system to address hydrocarbon impacted groundwater at and in <br /> vicinity of the subject site. The scope of work included: <br /> • updating the site specific health and safety plan, <br /> • obtaining the required well and sparge point installation permits, <br /> • installing four off-site groundwater monitoring wells, <br /> • installing 15 ozone sparge points, <br /> • installing an ozone microsparge system, <br /> • surveying the Top of Casing(TOC) elevations for the new monitoring wells, <br /> • developing and sampling the new wells, <br /> • collecting and submitting selected soil and groundwater samples for chemical analysis, <br /> • arranging for a ConocoPhillips contractor to dispose of the waste materials, and <br /> • preparing a report presenting the observations associated with the well installation and the <br /> sparge system startup. <br /> The scope of work was originally proposed in GR's Work Plan For Additional Delineation and <br /> Ozone Microsparge System Installation, dated September 19, 2002. This work was approved by <br /> the San Joaquin County Public Health Services, Environmental Health Division (PHS-EHD) in a <br /> letter to ConocoPhillips dated September 25,2002. <br /> 140193.14-2 <br />
