PERMISSIONISHEREBYGRANTED TO 2.Attach two(2)sets ofplans orsketches, ifopplicable. Install 2 monitoring wells using hollow stem auger in the City Right-of-Way on the frontage road north of Pine Street and south of Highway 99(see Attachment 1). One well will be installed to a depth of 70 feet bgs. The second well will be drilled near the first well and drilled to a depth of approximately 120 feet bgs. These wells will be known as M-56A and M-57A. Borehole diameter for both wells will be 8",with a 2"well completed in each borehole.Flush mount well cover installation,monument and well box will be placed over each well. Soil and water from the drilling and development activities will be contained and disposed of at a location owned by the applicant.Traffic control will be completed based on the traffic control plan employed by ECM(see Attachment 2). Cost of ROW work is included as Attachment 3 and the City is named as additional insured in Attachment 4.
