APR-OS-H / WO :02 FRum; FULSUM 5Uu im �'in� rKuJ . a u l 5 i w J eJ4 Assessor's Parcel No.. 023-190-01 PERMIT TO ENTER FOR GEOTECBMCAL SAMPLING FOR FOLSOM SOII'If CANAL CONNECTMON)PROJECT For a consideration of$500 per occurrence, permission is granted to East Bay Municipal Utility District ("District") for a period between December 1, 1996 through March 31, 1997 to enter in, over, and upon owner's land to bore hole(s) and excavate test pit(s) as delineated on the map attached hereto and made a part hereof for the purposes of geotechnical field exploration in connection with the District's Initial Study of the Folsom South Canal Connection Project. Geotechnical sampling will be either bore hole(s) or test pit(s). The exploration hole(s) will be approximately 30-50 feet in depth. Test pits will be approximately eight(8) feet wide, 20 feet long, and 15 feet deep_ Test pit(s)will be bacirgled_ After sample collecting is complete, bore hole(s)will be backfilled and sealed with grout_ Equipment to be used for the exploration includes a truck mounted drill rig, a water truck, backhoe and a pick-up truck. The rights and privileges hereby granted to District may, at the option of the District, be exercised by any authorized agent or contractor of District, including CH2M Hzll, Olivia Chen Consultants, and Pitcher Drilling. District agrees upon the completion of any work hereunder to restore the surface of the ground as near as possible to the condition in which it was prior to the commencement of said work_ 1t is understood and agreed that the District will leave the premises in a clean and orderly condition and will repair, replace, or compensate for any existing improvements that may have been disturbed or removed during the course of work_ Project team members and the project consultant have been thoroughly instructed in proper field procedures and conduct. The following rules will be followed: • Property owners will be given at least 24 hours notice prior to entry by phone or mail. Crates will be left as they were found. • No pets or firearms will be permitted. All machinery and vehicles will be equipped with spark arrestors_ All vehicles will be required to carry fire extinguishers and shovels. Smoking is permitted only in enclosed vehicles. • No trash or other evidence of field visits will be left on the property. • All field staff will carry identification and information regarding this project. • Vehicle speeds will be kept to five (5) miles per hour on unpaved roads in agricultural properties to minimize dust. Owner is welcome to accompany or observe any of the crews working on their property at any time. i
