APR-OS-H / WO :02 FRum; FULSUM 5Uu im �'in� rKuJ . a u l 5 i w J eJ4 <br /> Assessor's Parcel No.. 023-190-01 <br /> PERMIT TO ENTER <br /> FOR GEOTECBMCAL SAMPLING <br /> FOR FOLSOM SOII'If CANAL CONNECTMON)PROJECT <br /> For a consideration of$500 per occurrence, permission is granted to East Bay Municipal Utility <br /> District ("District") for a period between December 1, 1996 through March 31, 1997 to enter in, <br /> over, and upon owner's land to bore hole(s) and excavate test pit(s) as delineated on the map <br /> attached hereto and made a part hereof for the purposes of geotechnical field exploration in <br /> connection with the District's Initial Study of the Folsom South Canal Connection Project. <br /> Geotechnical sampling will be either bore hole(s) or test pit(s). The exploration hole(s) will be <br /> approximately 30-50 feet in depth. Test pits will be approximately eight(8) feet wide, 20 feet long, <br /> and 15 feet deep_ Test pit(s)will be bacirgled_ After sample collecting is complete, bore hole(s)will <br /> be backfilled and sealed with grout_ Equipment to be used for the exploration includes a truck <br /> mounted drill rig, a water truck, backhoe and a pick-up truck. <br /> The rights and privileges hereby granted to District may, at the option of the District, be exercised <br /> by any authorized agent or contractor of District, including CH2M Hzll, Olivia Chen Consultants, and <br /> Pitcher Drilling. <br /> District agrees upon the completion of any work hereunder to restore the surface of the ground as <br /> near as possible to the condition in which it was prior to the commencement of said work_ <br /> 1t is understood and agreed that the District will leave the premises in a clean and orderly condition <br /> and will repair, replace, or compensate for any existing improvements that may have been disturbed <br /> or removed during the course of work_ <br /> Project team members and the project consultant have been thoroughly instructed in proper field <br /> procedures and conduct. The following rules will be followed: <br /> • Property owners will be given at least 24 hours notice prior to entry by phone or mail. <br /> Crates will be left as they were found. <br /> • No pets or firearms will be permitted. <br /> All machinery and vehicles will be equipped with spark arrestors_ <br /> All vehicles will be required to carry fire extinguishers and shovels. <br /> Smoking is permitted only in enclosed vehicles. <br /> • No trash or other evidence of field visits will be left on the property. <br /> • All field staff will carry identification and information regarding this project. <br /> • Vehicle speeds will be kept to five (5) miles per hour on unpaved roads in agricultural <br /> properties to minimize dust. <br /> Owner is welcome to accompany or observe any of the crews working on their property at any time. <br /> i <br />
