Anderson and Associates. The report was submitted for <br /> replacement of Well MW-3, which is believed to have been constructed improperly allowing grout to <br /> enter the well. Installation of groundwater monitoring wells at the facility was required by the 26 August <br /> 2004 California Water Code Section 13267 Order; further investigation of site conditions and <br /> replacement of Well MW-3 was required by a 22 March 2005 Notice of Violation(NOV) for submittal <br /> of an inadequate groundwater monitoring well installation report. The 22 March 2005 NOV required the <br /> following: <br /> 1. Installation of a replacement well for Well MW-3; <br /> 2. Destruction of Well MW-3; <br /> 3. Investigation of subsurface conditions at Wells MW-1,MW-2, and MW-3; and <br /> 4. A project schedule. <br /> In general the 11 May 2005 Workplan addresses the items listed above. However, some clarification of <br /> the workplan elements is needed. In addition,the document relies upon previously submitted reports to <br /> describe planned field activities. Such reliance makes errors by field personnel more likely. Staff would <br /> prefer to see one workplan that can be relied upon by field personnel rather than referencing several <br /> documents. <br /> I have the following comments on the 11 May 2005 Workplan: <br /> 1. The abandonment of Well MW-3 describes pouring grout mixture into the boring if it is dry. This <br /> is not acceptable. All grout must be placed using a tremie pipe. <br /> 2. The material to be used for grout is not described. Please state what material will be used to grout <br /> the annular space,the abandoned Well MW-3, and the soil borings. <br /> 3. Please present a well construction detail for proposed Well MW-5. <br /> 4.
