Focused Groundwater Investigation Work Plan <br /> Deuel Vocational Institution(DVI), Tracy, California Data Acquisition and Quality Assurance/Quality Control <br /> 5.5.3 Field Data and Sample Documentation Procedures <br /> Field personnel will be responsible for the use and maintenance of field notebooks when conducting <br /> project-related field work. Field notebooks provide a means for recording all data collection activities <br /> performed at a project site. Field notebooks are intended to provide sufficient data and observation notes <br /> to enable participants to reconstruct events that occur during site activities. All entries need to be as <br /> factual, detailed, and descriptive as possible, so that a particular situation can be reconstructed without <br /> reliance on the collector's memory. Field notebooks are not to be used as a sole source of project or <br /> sampling information,nor should they be used for recording opinions or any other inappropriate <br /> terminology. Field notebooks will be completed with consecutively numbered pages.Notebooks will be <br /> permanently assigned to field personnel. <br /> The cover of each notebook will include the following information: <br /> • person or organization to whom the book is assigned, <br /> • book number, <br /> • project number(if different from site number), <br /> • site name and number, and <br /> • start date of notebook entries. <br /> Entries into the notebook may contain a variety of information. At a minimum,notebook entries must <br /> include the following information at the beginning of each day: <br /> • date, <br /> • start time, <br /> • weather conditions, <br /> • county, state,and site address, <br /> • all field personnel present and directly involved, and <br /> • level of personal protection being used on site. <br /> In addition, information recorded in the field notebook should include the following: <br /> • a detailed description of sampling locations,physical parameters, and GW level measurements; <br /> • information on field QC samples(i.e., duplicates and trip blanks); <br /> • observations about the site and samples (e.g., odors, appearance); <br /> • information about any activities extraneous to sampling activities that may affect the integrity of <br /> the samples (e.g., low-flying aircraft nearby, fossil-fueled motors being used nearby,painting <br /> operations being carried out upwind from sampling sites); <br /> • equipment used on site, including time and date of calibration(equipment calibration also will be <br /> recorded in the calibration logbook); <br /> • maps or photographs acquired or taken at the sampling site; and <br /> • forms used during sampling. <br /> All notebook entries will be made in indelible black or blue ink.No erasures will be permitted. If an <br /> incorrect entry is made,the data will be crossed out with a single strike mark and will be dated and <br /> initialed by the originator. Entries will be organized into easily understandable tables, if possible. The PM <br /> or designee will review field notebooks that result from field operations for completeness and accuracy, <br /> on completion of the field work. <br /> 5-5 <br />
