ENGEO <br /> INCORPORATED <br /> Safety <br /> Each laboratory will be responsible for maintaining a current Laboratory Safety Manual (LSM) <br /> in compliance with Occupational Safety and Health Administration (OSHA), or equivalent state <br /> or local regulations. The laboratory shall be operated and maintained in a safe manner, and <br /> provide a working environment for staff that has as its top priority the implementation of any and <br /> all measures necessary for the highest level of protection of employee health and safety. <br /> REPORTING <br /> Summary reports will be prepared at the conclusion of the monitoring well installation and initial <br /> sampling event, and after each subsequent monitoring event. The reports will contain a summary <br /> of test results as presented by the laboratory in standard units and will be forwarded to other <br /> consultants as requested for review and analysis. Deviations from standard operating <br /> procedures, sample handling, etc. will be reported in the summary. <br /> The monitoring well installation and initial sampling report will contain the following <br /> information: <br /> • General Information: <br /> o Purpose of the well installation project <br /> o Brief descriptions of the local geology and hydrogeology, including descriptions <br /> of subsurface conditions encountered <br /> o Number of wells installed and copies of county well construction permits <br /> o Topographic map also showing cultural features <br /> o Detailed scaled site layout <br /> • Drilling Details: <br /> o On-site supervision of drilling and well installation activities <br /> o Drilling contractor and driller's name <br /> o Description of drilling equipment and techniques <br /> o Equipment and decontamination procedures <br /> o Soil sampling intervals and logging methods <br /> 4943.4.640.01 <br /> April 17, 2007 17 <br />
