Missing from the site plan is (L) in diesel turbine sump <br /> designating PLLD. A site plan must be submitted identifying the locations where monitoring will be performed. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload <br /> a copy of the UST Monitoring Site Plan. <br /> This is a repeat violation, Class II. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to check that the alarms were responded to appropriately. The L10: 91 STP annular <br /> space alarm on 1/21/16 was addressed by the designated operator in the 1/27/16 designated operator report; <br /> however, in the designated operator monthly reports for 2/23/16, 3/29/16, 4/26/16, and 5/24/16, the designated <br /> operator made note that L10:91 STP annular space is still in alarm but did not address the problem. The designated <br /> operator noted in the 3/29/16 designated operator monthly inspection report that S2: vapor communication alarm had <br /> occurred on 3/13/16. There was no record of this in the panel printout. The designated operator noted S4: 87-91 <br /> vent no vacuum alarm on 7/23/16 and stated that S4 was still in alarm but did not address the problem. During the <br /> monthly inspection, the designated operator shall review the alarm history for the previous month, check that each <br /> alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation <br /> taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 305 HSC 25290.1(e) VPH monitoring of the interstitial spaces of the UST system is not maintained. <br /> Communication in under dispenser containment(UDC) 5/6 failed when tested. The interstitial space of UDC 5/6 shall <br /> be maintained, as designed, under constant vacuum such that a breach in the primary or secondary containment is <br /> detected before the hazardous substance is released into the environment. Communication in the 87-, 91-, and <br /> diesel product fill sumps, as well as vacuum sensor monitoring of the secondary containment piping of the <br /> vacuum/pressure/hydrostatic (VPH) system were not tested during the inspection today. The interstitial space of the <br /> UST shall be maintained, as designed,under constant vacuum such that a breach in the primary or secondary <br /> containment is detected before the hazardous substance is released into the environment. Immediately contact a <br /> properly licensed, trained, and certified contractor to test communication of the VPH components listed above under <br /> inspection by the Environmental Health Department(EHD). <br /> This is a repeat violation, Class II. <br /> Page 4 of 5 <br />
