This <br /> permit has been Amended to install a gasket on the Vent Box lid on the underground storage <br /> tank (UST) system at the above noted address. The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Fatinah Zareef at (209) 953-7311 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. Sensor functionality: Functionality of the monitoring system must be verified to <br /> ensure that monitoring has been restored to meet the requirements of Title 23 <br /> California Code of Regulations and the Health & Safety Code. If it is determined that <br /> a cold start is necessary, a copy of the alarm history shall be printed prior to <br /> performing the "cold start" of the Veeder Root system and made available at the time <br /> of EHD inspection. <br /> b. Before backfill of excavation and after the interstitial fluid is restored a visual <br /> inspection of the repaired diesel sump annular space is required to verify no brine <br /> leaks are present. <br /> c. A visual inspection of the Vent Box to make sure no liquid is inside and to make sure <br /> gasket was installed. <br />
