This is a revised approval letter reflecting the <br /> permit application addendum received on August 29, 2013 for utilization of Bravo penetrations <br /> and addition of a cold start to the existing SR0067629. The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Elena Manzo at (209) 953-7699 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). <br /> a. Sensor functionality: Functionality of the monitoring system for all sensors must be <br /> verified to ensure that monitoring has been restored to meet the requirements of Title <br /> 23 California Code of Regulations and the Health & Safety Code. A copy of the <br /> alarm history shall be printed prior to performing the "cold start" of the Veeder Root <br /> system and made available at the time of EHD inspection. <br /> b. Leak detector test: all electronic line leak detectors must be tested to confirm that the <br /> device can detect a leak of 3 gallons per hour at 10 psi. <br /> c. Before backfill of the excavation and after the interstitial fluid is restored a visual <br /> inspection of the repaired sumps will be required <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br />
