Neil O.Anderson&Associates HASP Page 16 of 21 <br /> March 18,2014 <br /> ' 10.3 Injury/Illness Reporting <br /> OSHA Records Required <br /> ' NOA will maintain copies of required accident investigations, copies of the OSHA 301 and 300 <br /> Logs, hazard identification and abatement, and certification of employee safety training. A <br /> written report will be maintained on each recordable accident, injury or on-the-job illness. See <br /> ' Attachment D for Accident Report Form. A record of each such injury or illness is recorded on <br /> OSHA Log and Summary of Occupational Injuries Form 300 according to its instructions. <br /> Supplemental records of each injury are maintained on OSHA Form 301, or state-specific <br /> Employers Report of Injury or Illness Form. Every year, a summary of all reported injuries or <br /> illnesses is posted no later than February 1, for three months, until May 1, on OSHA Form 300A. <br /> These records will be maintained by the contractor for five (5) years from the date of <br /> ' preparation. <br /> The results of each investigation will be presented in writing and submitted for review to NOA <br /> t or its insurance risk management advisors, and if the accident resulted in serious injury, to <br /> company attorneys. All accidents involving a death, multiple personnel injuries, lost time injury, <br /> or substantial property damage will be investigated. <br /> The procedure may be directed by the attorneys to provide the most reliable evidence or <br /> description legally permissible. All investigations pursuant to the directions of legal counsel will <br /> be protected by all applicable privileges. The attorney will provide more detail on this topic <br /> during the investigation. <br /> 11.0 CONFINED SPACES <br /> No confined space entry is anticipated at the site. A confined space protocol will be developed <br /> for Agency review and approval should conditions at the site change. <br /> 12.0 EXPOSURE MONITORING PLAN <br /> Procedures for monitoring exposures to site workers and the community are discussed in the <br /> following sections. <br /> 12.1 Air Monitoring <br /> NOA will implement appropriate procedures to control the generation of airborne dusts by soil <br /> removal activities. Such procedures will include but will not be limited to the following: <br /> • Liberation of dust during the removal operations will be minimized as necessary with the <br /> use of water as a dust suppressant. The water will be trucked onto the Site for use in <br /> dust control activities only. NOA will control dust generation by spraying water prior to <br /> daily work activities, during excavation/loading activities (as necessary to maintain <br /> concentrations below action levels), and at truck staging locations.
