(WHF) for the above referenced site. PHS/EHD has the following <br /> comments: <br /> • A survey that identifies all wells located within a 2000-foot radius of the site should be conducted <br /> before the commencement of any work on site. A report of'the survey should include a map' <br /> I <br /> g the location of all wells and any known wn construction details of the wells identified. The <br /> n <br /> I, <br /> well survey report should be submitted-to PHS/EHD by June 5, 1999. <br /> • An additional monitoring well (MW)should be installed in the dispenser area by Soil Boring#1 <br /> (SB-1) where the water sample with the'highest concentration of Methyl Tert-butyl Ether(MTBE) <br /> was collected. <br /> • Soil samples should also be collected at the capillary fringe and at each significantly different <br /> lithologic unit in addition to every interval of 5 feet. <br /> • An improper cement/bentonite mixture was used to grout the soil borings at the site on October 5, <br /> 1998.
