ADDITIONAL INFORMATION REQUIREMENTS <br /> LOST ISLE RESORT, ACKER ISLAND <br /> SAN JOAQUIN COUNTY <br /> Please provide the following information in the form of a technical report prepared and signed by a <br /> California licensed engineer: <br /> 1. A description of the site that includes a site vicinity map, description of buildings <br /> (restaurants/bars/etc.), public facilities (toilets/showers/etc.), caretaker residence, attendance <br /> estimates for on and off season, special events that draw large numbers of attendees, pump out <br /> facilities, etc.; <br /> 2. A narrative description of the sources and types of wastewater flowing into the system, flow rates, <br /> the capacity of the system, and applicable factors of safety (with justification). The description <br /> should present the flow data gathered in 2004; <br /> 3. A detailed narrative description of all proposed on-site wastewater treatment, conveyance, storage, <br /> pumping, and disposal systems planned. If pit toilets are contemplated, describe how wastewater <br /> will be transported from the toilet to the storage facility; <br /> 4. A description of automated controls and safeguards to prevent spills. Also describe how odors will <br /> be controlled; <br /> 5. A description of the proposed flow rate monitoring system; <br /> 6. Chemical characterization of the wastewater quality and documentation of acceptance of the waste <br /> by a permitted wastewater treatment system; <br /> 7. Describe how stormwater that falls on the storage systems will be handled. A water balance is <br /> required to demonstrate adequate capacity. <br /> 8. A description of the potable water supply (sources and rate of delivery); <br /> 9. Provide an evaluation of the collection system that addresses infiltration and inflow as well as <br /> leakage out of the collection system; <br /> 10. A copy of the certified document prepared pursuant to the California Environmental Quality Act <br /> (CEQA); <br /> 11. A description of operation and maintenance procedures to be employed; <br /> 12.
