Maintenance records for the following alarms were not found on site. S4 - 87/91 vents vacuum <br /> sensor went into communication alarm on 12-5-09, 2-8-10 and 2-3-10. S1 - 87 line vacuum sensor <br /> went into communication alarm on 4-23-10. These records shall be kept on site for at least 3 years. <br /> Immediately locate and maintain the maintenance records on site and submit copies to the EHD by <br /> September 10, 2010. <br /> 23c. The designated operator failed to document all the alarms for the previous month and check that <br /> they were responded to appropriately on the 2-10, 12-09 and 4-10 designated operator monthly <br /> inspection reports. The missing alarms include: S4 - 87/91 vents vacuum sensor went into <br /> communication alarm on 12-5-09, 2-8-10 and 2-3-10. S1 - 87 line vacuum sensor went into <br /> communication alarm on 4-23-10. During the monthly inspection, the designated operator shall review <br /> the alarm history for the previous month, check that each alarm was documented and responded to <br /> appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this <br /> facility are including all of the required information on the reports. <br /> 47. The S5 - Diesel line high liquid sensor failed when tested. The interstitial space of the UST system <br /> shall be maintained, as designed, under constant vacuum such that a breach in the primary or <br /> secondary containment is detected before the hazardous substance is released into the environment. <br /> Correct immediately by having a properly licensed, trained, and certified contractor replace the failed <br /> component with a functional component (LG 113-x listed, if applicable) and obtain a permit within one <br /> business day from the EHD. If the failed component can't be replaced immediately, there is a possibility <br /> that the Diesel UST system may be red tagged by the EHD to prevent fuel inputs. <br /> Notes: <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by September 10, 2010. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE AB VE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION A NY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspe Rea .ved B Title: <br /> EH -02-003 Rev 06/01/10 CONTINUATION FORM <br />
