/ 9000562C, Page 10 <br /> ( B.6.7 Reports <br /> B.6.7.1 Health and safety infractions and accidents resulting in illness or injury or <br /> property damage must be reported to WCC health and safety authorities. Reporting <br /> procedures are described in Health and Safety Operating Procedure No. HS-502. <br /> B.6.7.2 Upon completion of the work described in this HSP, a safety completion <br /> report must be submitted to the HSO. The report shall contain an evaluation of this HSP <br /> and a summary of health and safety activities,including airborne chemical monitoring data, <br /> decisions based on the data, and names of WCC and subcontractor personnel who worked <br /> or visited the work site. <br /> B.6.8 Work Zones <br /> B.6.8.1 Before intrusive operations begin, an exclusion zone must be established <br /> around the drill rig after it has been positioned. The boundary of the exclusion zone shall <br /> extend at least 25 feet from the drill rig and must be marked with traffic cones/tubes or <br /> barriers spaced no more than three feet apart or with stakes spaced no more than 20 feet <br /> apart connected with fluorescent orange tape. The size and location of the exclusion zone <br /> may be altered due to obstacles beyond control (such as buildings, landscaping, etc.) The <br /> site safety officer shall decide where to locate the exclusion zone. Only authorized <br /> personnel will be allowed to enter the exclusion zone. "Authorized Personnel Only Beyond <br /> This Point" signs or equivalent shall be prominently posted along the boundaries of the <br /> exclusion zone. <br /> B.7 EMERGENCY PROCEDURES <br /> B.7.1 In the event of an accident or incident requiring onsite assistance, contact the <br /> following applicable numbers. All accidents shall be formally reported using the Accident <br /> Investigation Report, ENG Form 3394. <br /> 9222- Ambulance, 7:30 a.m. to 4:00 p.m. <br /> 6911- Ambulance, 24-hour emergency <br /> 9333- Depot Fire <br /> 9302- Depot Police <br />
