502 . 3 REPORTING PROCEDURES <br /> 502 . 3 . 1 REPORTING FORMAT <br /> Incident reports shall be prepared by completing Form HS- <br /> 502 . This form may be obtained from any WCC health and safety <br /> officer. <br /> 502 . 3 . 2 RESPONSIBLE PARTY <br /> Reports of incidents occurring in the field shall be <br /> prepared by the site safety officer or, in the absence of the site <br /> safety officer, the supervising field engineer, witness, or <br /> injured/exposed individual . <br /> 502 . 3 . 3 FILING <br /> A report must be submitted to the health and safety <br /> officer of the business unit to which the project manager belongs <br /> within 24 hours of each incident involving medical treatment. In <br /> turn, the health and safety officer must distribute copies of the <br /> report to the corporate health and safety administrator and the <br /> corporate health and safety officer. When an injury or illness is <br /> reported, the business unit health and safety officer must deliver <br /> a copy of the report to the individual in charge of personnel <br /> affairs so that a Worker' s Compensation Insurance Report can be <br /> filed if necessary.
