CLEANUP AND ABATEMENT ORDER NO.R5-2002-0718 -5- <br /> CHEVRON <br /> 5-CHEVRON PRODUCTS COMPANY <br /> • BANTA BULK FUEL TERMINAL <br /> BANTA, SAN JOAQUIN COUNTY <br /> 6. Within 90 days of submittal of the final Site Assessment Report, submit a Site Conceptual Model <br /> with a profile of the site, characterizing the soil and.groundwater pollutants, and summarizing all <br /> finvestigative and remedial activities the Discharger has conducted. <br /> FEASIBILITY STUDY AND CLEANUP <br /> 7. By 4 April 2003, submit an evaluation report of the ISO-GEN remediation system, including the <br /> feasibility of continued operation or the proposal for alternative and/or additional remedial options. <br /> 8. Within 60 days of staff concurrence with the ISO-GEN evaluation report, submit a Feasibility <br /> Study/Remedial Options Evaluation Report for soil and groundwater remediation. The report shall <br /> contain the information in Attachment 4,which is made part of this Order. The proposed preferred <br /> alternative must meet the range of cleanup levels as described in the Basin Plan and Resolution No. <br /> 92-49. The Discharger shall attempt to clean up each constituent to background concentrations, or <br /> to the level that is technically and economically feasible and at least achieves the WQOs of the <br /> Basin Plan, listed in Finding 9. <br /> 9. Within 90 days of staff concurrence with the Feasibility Study/Remedial Options Evaluation Report <br /> for soil and groundwater remediation, submit a Cleanup Plan,which describes the approved <br /> alternative(s) for remediation and includes a time schedule to conduct the remediation activities. <br /> The approved time schedule to implement the remediation shall become a part of this Order. <br /> 10. Within 60 days of Executive Officer approval of the Cleanup Plan for groundwater remediation, <br /> ' commence remediation or installation of the remediation system. The Discharger shall notify staff a <br /> minimum of 72 hours prior to beginning field work. <br /> r11. According to the Executive Officer's approved time schedule for construction and implementation <br /> of the Cleanup an, P Plan, submit a report describing the status and results of the remedial work <br /> (Cleanup Implementation Report). The report shall also clearly show whether the installation of <br /> any remediation system is complete, and if not, give a schedule and proposed work plan for <br /> installation of the remaining remedial activities, including a proposed monitoring plan. <br /> GENERAL REQUIREMENTS <br /> 12. Continue to conduct monitoring of the existing wells and any additional wells in accordance with <br /> MRP No. 98-801 or any revised MRP is issued by the Executive Officer, <br /> 13. Continue to reimburse the Regional Board for reasonable costs (as.estimated by Regional Board <br /> staff at the beginning of each fiscal year) associated with oversight of the cleanup of this facility. <br /> Failure to do so shall be considered a violation of this Order. <br /> ' 14. Conduct work only after work plans are approved or conditionally approved by Regional Board <br /> staff. <br /> • 15. Include in all reports a cover letter from the Discharger stating concurrence or nonconcurrence with <br /> the conclusions and recommendations in the report. <br />
