Hutchins St. <br /> Lodi, CA 95201 <br /> On August 22, 1996, San Joaquin County Public Health Services, Environmental <br /> Health Division (PHS-EHD) received the "Case Closure Report" dated August 1996, as <br /> submitted by Daniel, Mann, Johnson & Mendenhall (Randolph Stone CEG #1214) and <br /> has the following comments. <br /> Established policy and contractual agreements require that PHS-EHD staff review all <br /> closure documents and evaluate their completeness and adherence to closure <br /> requirements listed within Appendix B of the Tri-Regional Guidelines. As soon as the <br /> accepted report is reviewed and the PHS-EHD case manager agrees that the site <br /> qualifies for closure, a Case Closure Summary and a complete site chronology is <br /> prepared. These documents, along with a recommendation, are forwarded to the <br /> Central Valley Regional Water Quality Control Board (CVRWQCB) for evaluation. The <br /> CVRWQCB has thirty (30) days to review and prepare comments on the site. <br /> PHS-EHD will not be able to grant the site conditional closure by September 23,1996 <br /> as requested in your August 21, 1996 correspondence. <br /> PHS-EHD will make every attempt to review your Closure Report as soon as possible. <br /> If the Closure Report submitted is complete, your site could be placed on the October <br /> 24, 1996 Technical Review Committee (TRC) meeting agenda for evaluation. You will <br /> be contacted once the Closure Report is accepted. Your environmental consultant will <br /> be allowed to present your site to the TRC for conditional closure evaluation. <br /> Once it is determined that the site should be conditionally closed, the monitoring and <br /> vapor wells must be destroyed under a permit issued by PHS-EHD. After the <br /> destruction of the wells, the final conditional closure letter will be issued. <br /> A Division of San Joaquin County Health Care Services <br />
