A drilling permit application and a permit application fee of $89 shall be submitted to <br /> PHS/EHD and approved by PHS/EHD before initiating any on site drilling. PHS/EHD <br /> shall be notified a minimum of 48 hours in advance of scheduled sampling activities. <br /> 2 . One monitoring well shall be installed within 10 feet of the former nested tank location <br /> in a down gradient position. <br /> 3.
