Frontier Transportation, Inc. <br /> Health and Safety Plan <br /> Page 2 <br /> 3, RESPONSIBILITIES <br /> 3.1 HEALTH AND SAFETY COORDINATOR <br /> As BEA's Health and Safety Coordinator (HSC), James Jazmin is responsible for directing and <br /> implementing the HASP and ensuring that all BEA and subcontractor personnel have been trained in HASP <br /> procedures. The HSC will coordinate safety activities with subcontractors and serve as liaison with public <br /> officials who might wish to monitor health and safety activities on-site. The HSC will also ensure that proper <br /> protective equipment is available and used in the correct manner, that decontamination activities are carried <br /> out correctly, that specific site hazards are noted and accounted for in the Work Plan and that employees have <br /> knowledge of the local emergency medical system. The HSC may conduct periodic site audits to ensure <br /> compliance with the HASP and to note any additional hazards or concerns . The HSC has stop-work <br /> authorization, which shall be executed upon determination that an imminent health or safety hazard exists. <br /> 3.2 DESIGNATED SITE SAFETY OFFICER <br /> As BEA's Site Safety Officer (SSO), Windy Brown is responsible for implementing the site-specific <br /> HASP in the absence of the HSC. The SSO shall conduct daily tailgate safety meetings and ensure that only <br /> authorized personnel are allowed at the site. In addition, the SSO shall ensure that the daily sign-in logs for <br /> site persons and visitors are maintained. The SSO shall report any unsafe acts or conditions to the HSC . The <br /> SSO also has stop-work authorization, which shall be executed upon determination that an imminent danger <br /> to life or health exists . If a stop-work order is issued, due to safety concerns, the HSC shall be contacted <br /> immediately and appropriate steps taken to correct the situation. <br /> 3.3 PROJECT MANAGER <br /> BEA Project Manager, James Jazmin is the direct link between BEA and Mr. Paul Zumberge. He is <br /> responsible for directing all on-site operations, including the overall implementation of the Health and Safety <br /> Program. In addition, the Project Manager is responsible for ensuring that adequate resources and personnel <br /> protective equipment are allocated for the health and safety of site personnel. The Project Manager is also <br /> responsible for ensuring that the safety personnel (via the HSC) are given free access to all relevant site <br /> information that could impact health and safety. He will correct conditions or work practices that could lead <br /> to employee exposure to hazardous materials. <br /> 3.4 OCCUPATIONAL MEDICAL CONSULTANT <br /> SAN BERNARDINO COMMUNITY HOSPITAL, BEA's Occupational Medical Consultant, will be <br /> available to answer medical questions and provide guidance in unexpected situations . The Medical <br /> Consultant will recommend appropriate medical monitoring for the site team members . <br /> 3.5 EMPLOYEES <br /> All BEA employees working at the site are responsible for reading and understanding the HASP. <br /> They will be held accountable for complying with all aspects of the HASP. <br />
