Frontier Transportation, Inc. <br /> Health and Safety Plan <br /> Page 9 <br /> 9.2 .2 EMERGENCY DECONTAMINATION <br /> In the event that a seriously injured person is suspected of being contaminated, the SSO or other site <br /> worker will wrap the injured individual in clean plastic sheeting to prevent contamination of the ambulance. <br /> Less severely injured individuals will have their protective clothing carefully cut off before transport to the <br /> hospital. <br /> 9.2.3 COVERALLS <br /> If coveralls are sent off-site for cleaning, the cleaner establishment will be notified of any hazards <br /> prior to receiving the coveralls. <br /> 9.3 WATER AVAILABILITY <br /> Potable water will be available on-site. <br /> 9.4 RECORD KEEPING <br /> To assure HASP implementation, many site activities will be documented. These include <br /> maintenance of the HASP at the site; employee HASP sign-off; daily safety briefings; site sign-in log; <br /> emergency medical data sheets; health and safety log-notes (which include instrument calibration records, <br /> sampling data, monitoring results and incident reports); chemical safety data sheets; and other records <br /> identified in the HASP. All documents noted are subject to audit and review by the Project Health and Safety <br /> Coordinator and/or Certified Industrial Hygienist. <br /> 9.5 EMERGENCY RESPONSE PLAN <br /> Emergency response procedures have been developed for extraordinary events that could occur <br /> during field operations. These events include accidents and/or injuries, chemical exposure, spills and fires. <br /> In general, the following actions shall be implemented in the event of an emergency: <br /> 1 . First aid or other appropriate initial action will be administered by those closest to the <br /> accident/event. This assistance will be coordinated by the designated Site Safety <br /> Officer and will be conducted so that those rendering assistance are not placed in a <br /> situation of unacceptable risk. The primary concern is to avoid placing a greater number <br /> of personnel injeopardy. <br /> 2 . The Project Manager, Field Supervisor and Health and Safety Coordinator will be notified <br /> immediately. They will in turn notify the Mr. Paul Zumberge. <br /> 3 . An AccidentAncident Report will be completed by the injured individual or witness and Site <br /> Supervisor. The Accident Report will then be forwarded to the Project Manager. Upon <br /> reviewing and commenting on the accident/incident, the form will be forwarded to the BEA <br /> Health and Safety Coordinator who in tum will investigate and make comments on the <br /> accident/incident. Any necessary changes to the operation will be made to prevent the same <br /> accident or near miss situation from occurring in the future. <br />
