04 1 <br /> Frontier Transportation, Inc. <br /> Health and Safety Plan <br /> 1 Page 1 <br /> 1. HEALTH AND SAFETY PLAN <br /> I Biophysics Environmental Assessments, Inc. (BEA), has established this site-specific Health and Safety Plan <br /> (HASP) as pail of he work plan for all individuals engaged in field assessment activities at the Frontier Transportation, <br /> Inc. site located at 425 Larch Road, Tracy, California. All site work shall be conducted in a safe manner and comply <br /> with EPA, state and local regulations, in particular OSIIA 29 CFR, part 1910, and California Administrative Code Title <br /> 8. In addition, all site work will comply with BEA Corporate Health and Safety Program and all supporting Standard <br /> Operating Procedures. This HASP may be modified during actual field activities, if necessary, as more information and <br /> r site-specific data is obtained. <br /> Prior to any work on-site, an approved copy of this HASP (latest edition) shall be provided to all employees <br /> _ and subcontractors by the Project Manager. Each subcontractor will he responsible for providing their own HASP. At a <br /> minimum the subcontractors' HASP must meet the requirements of this HASP. BEA will review and approve each <br /> subcontractor HASP prior to initiation of field work <br /> i- 1.1 PURPOSE AND OBJECTIVES <br /> The purpose of this site-specific HASP is to provide guidelines and procedures to ensure the health and <br /> physical safety of those persons working at the Frontier Transportation, Inc. property. While it may be impossible to <br /> eliminate all risks associated with site work, the goal is to provide precautionary and responsive measures for the <br /> protection of on-site personnel, the general public and the environment. The HASP objectives are as follows: <br /> * Ensure the safety of all site personnel <br /> J * Protect the public and the environment <br /> * Adhere to BEA Health and Safety procedures <br /> 1.2 IMPLEMENTATION <br /> This site-specific HASP, and any additions included in a subcontractor HASP, will be reviewed and the Field <br /> Team Review Form (Attachment 1) will be completed by all site personnel prior to their scheduled field work, <br /> j I Whenever the site-specific HASP is revised or amended, personnel will be instructed in the new procedures and <br /> required to complete a new Field Team Review Form. The site-specific HASP will be implemented in the field by <br /> BEA's Health and Safety Coordinator and/or designated Site Safety Officer. <br /> - 2.0 BACKGROUND <br /> 2.1 SITE LOCATION AND DESCRIPTION <br /> :1 <br /> The site is located at 425 Larch Road in Tracy, California and has operated as a truck yard. Petroleum <br /> hydrocarbon and oxygenate (MME) impacted soil and groundwater was identified at the site. <br /> - ' 2.2 SCOPE OFWORK <br /> Install two to three wells for the purpose of collecting environmental groundwater and soil samples in order to <br /> l:_I identify the extent of plume migration at the site. <br /> l 3* RESPONSIBILHUS <br /> 3.1 HEALTH AND SAFETY COORDINATOR <br /> I.J <br /> L[ <br />
