The form will contain the following <br /> information: <br /> * Cross-reference to the Chain-of-Custody Record; <br /> - II * Project name and number; <br /> ,l * Sample number; <br /> * Sample volume; <br /> * Preservative as required; <br /> * Requested testing program; <br /> * Required turnaround time; <br /> * Possible hazard identification; <br /> * Sample disposal requirements; <br /> The form will be signed and dated by the receiving laboratory sample management <br /> custodian. <br /> ll 6.2.2) Sample Identification <br /> Sample labels prevent the misidentification of samples. Following sample collection, <br /> labels will be affixed to each sample container.
