Audible visual only was selected for the overfill <br /> prevention-no Fill Tube Shut-Off Valve(although installed)was identified. No corrosion protection was identified as well <br /> for all USTs. Any change of information must be updated in CERS within 30 days of the changes. Immediately log <br /> into CERS, update the required information, and submit for review by the EHD. This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. <br /> The monitoring plan is not current and/or not approved by the EHD. Incorrect line leak detectors and UDC monitoring <br /> were identified in forms submitted into CERS. The monitoring plan must be uploaded to the California Environmental <br /> Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the <br /> EHD. This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br /> An accurate UST Monitoring Site Plan was not submitted, inaccurate UDC monitoring was identified (floats and chains <br /> as well as VR-208 were observed onsite)on the submitted map. A site plan must be submitted identifying the <br /> locations where monitoring will be performed. Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a repeat violation, Class II. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. <br /> This is a repeat violation, Class II. <br /> 209 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for employees onsite is not current; the most recent training date was <br /> listed as 4-28-15 for employees working today. The designated operator shall train facility employees for which he or <br /> she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training <br /> shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a Class II violation. <br /> Page 4 of 5 <br />
