The current year Certificate of Financial Responsibility does not have section B completed. Enter <br /> the tank operator name and upload into the California Environmental Reporting System (CERS). <br /> 105. The current 87 product tank form is incomplete. Add the fill tube shutoff valve in the overfill <br /> prevention section and submit the form in CERS. <br /> 106. The UST monitoring and response plans are incomplete or incorrect. Please see comments in <br /> CERS, correct and resubmit in CERS. The line leak detectors must be added to the monitoring plan. <br /> Section V of the response plan must be filled out. <br /> 201. The 5-14-13 monitoring system certification test results and an alarm log entry for a L3, 87 STP <br /> sump liquid sensor fuel alarm on 4-25-14 were not found on site. The alarm log entry should include the <br /> action taken in response to the alarm and include a copy of any service receipts. These records shall be <br /> kept on site for at least 3 years. Immediately locate and maintain the above listed documents on site <br /> and submit copies to the San Joaquin County Environmental Health Department (EHD) by June 20, <br /> 2014. <br /> 205. The August 2013 designated operator monthly inspection report was not found on site. <br /> Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br /> Locate and ensure that copies of the previous twelve months of designated operator monthly inspection <br /> reports are maintained on site.
