Secondary containment testing was performed on 05/21/12 and a test report has not been <br /> submitted to the EHD in a timely manner. A copy of the test report must be submitted within 30 days of <br /> the test. The test results were sent via email and received by EHD on 08/14/12, approximately 2 months <br /> late. Ensure test results are submitted within 30 days of the test. <br /> 109. Annual monitoring system certification, leak detector testing, and spill container test results were <br /> not been submitted to the EHD in a timely manner. The annual test was performed on 05/30/12 and a <br /> test report was received by EHD on 08/17/12, approximately 2 months late. The previous annual test <br /> was performed on 05/23/11 and its test results were received by EHD on 09/16/11, approximately 3 <br /> months late. A copy of the test report must be submitted within 30 days of the tests. Ensure test results <br /> are submitted within 30 days of the test. <br /> 113. A new Designated Operator Lyle Meeks from Franzen-Hill Inc. had started performing monthly <br /> visual inspection reports for February, March and April 2013 and no notification was provided to EHD <br /> within 30 days of changes shall be reported to the EHD within 30 days. Submit notification to the EHD <br /> identifying all current designated operators for this facility. <br /> 305. The Bravo box float and change in Dispenser 3/4 for 87 octane failed to stop the flow of product at <br /> the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and <br /> visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician <br /> adjusted and tighten the e4aage and verified functionality. <br /> 6� IN <br /> 311. The 91 mechanical line leak detector failed to detect a leak when tested. All line leak detectors shall <br /> be capable of detecting a 3 gallon per hour leak at 10 psi.
